students attending Public or Private school Grades K
to 8 will be on Saturday, September 6th from
9:00am to 12:00pm at the school office
Please pick-up a
Registration form from the Parish Office.

Emergency Contact Form
For transfer
students only - Permanent Record Card from
previous Religious Education
program

Fees: $45.00 for one child
$65.00 for two children
$80.00 for three children
$15.00 book fee per child
First Holy Communion 2009 Level 2 must have
completed Level 1 Confirmation 2009 Level 8 must
have completed Levels 3 to 7
